Immunization Records
A record of immunizations, signed by the administering physician, must be filed at the school before school opens or when enrollment occurs. West Virginia law requires the following:
DTP - Three (3) doses required, one of which must have been received after the 4th birthday.
DT may be accepted instead of DTP if a medical excuse is signed by a physician/health department representative or if the student is seven years of age or older.
Polio - Three (3) doses required, one of which must have been received after the 4th birthday.
MMR - Kindergarten Students - Two doses, first dose received not earlier than the first birthday. All other students - One dose received not earlier that the first birthday.
